Deathbed Smalls is a Leg Armor in Elden Ring. Deathbed Smalls is part of the Deathbed Set Armor Set, and seem to be cut from the final release of the game. Deathbed Smalls protect the player's legs by applying various defensive properties, it also changes the appearance as well when it is equipped. Some armor pieces may be available to both genders but may be slightly different for male and female characters.

In all fairness, the main reason they never officially added this into the game was probably because they only intended it to exist as part of Fia's appearance and they never made a version that fits the male character model, making for a bugged item.
As for WHY they never made a male version: well, Japan still has some interesting cultural (and a couple of legal) hangups when it comes to, shall we say, "effeminate" lifestyle choices.

Something to think about.... Fromsoft left these in purposely, yet because of how you could obtain these, they make it cut content (that also softbans you). If they didn't want people to have it, then WHY THE HIFL DON'T THEY REMOVE THE THING FROM THE GAME?! I don't understand why developers leave **** in if players aren't meant to have them. Seriously, either give the people what they want and spawn the item with the deathbed dress in the capital, or do the right thing and REMOVE THEM FROM THE GAME. The first thing I noticed when I found the dress was how bad it looked with my pants...
It was super disappointing because I stumbled on the dress and had been searching desperately the whole time for literally anything feminine.
You've got freaking bulge armor with a NPC who had two ball shapes on it even, ultra horn nipple armor and plenty of male clothing.
And even tight clothing female characters wear just looks like a skinny guy, the chest even gets COMPLETELY flattened.
It's just disappointing it's incomplete, you shouldn't have to scavenge for other similar ones and the fact other '' underwear '' leg pieces exist just begs the question why these don't.

As of 1.04 these can no longer be dropped for other players as the option to “leave” them has been greyed out.
You are still able to remove them from your inventory by selecting “discard”, but this deletes them entirely.
TLDR: It’s safe to pick up loot from strangers again.

Don't pick up any items dropped by anyone you don't know. FromSoftware seems to be penalizing people that have unobtainable items in their inventory regardless of if you yourself added the Deathbed Smalls into your own inventory or picked it up from someone who "kindly" left it for you.

PSA: Do not pick up anything from anyone you don't know because if you pick up an item that a hacker hacked in (like this item) then your character or even account may be flagged immediately afterwards and potentially be penalized just from picking up an item not able to be found normally.

I got summoned by a player, and when I joined their world, they dropped this item for me to pick up. Either it's obtainable somehow, or they cheated it in and gave it to me that way.

Wish this item was still in the game, the weight to defence ratio is pretty good. Also gives 66 vitality for 2 weight. Could have really helped many builds who play with one hand.

Console girls will have to make due with the zamor leggings and gold mask's leggings if we want to have gender affirming bottoms under our dresses.
