This helm oddly is crashing my game. I've been able to reproduce it three time trying to swap between the Cleanrot Helm and this instantly causes a freeze for some reason.

The knight archer by the altus highway junction drops the helmet and the greaves, still farming so I'll comment again if he drops anything else. He also can drop his greatbow.
Good farming spot located mostly west and a tiny bit north of the Altus Highway Junction grace in a camp surrounded by spiked logs. One knight on horseback and one sitting next to a fire. Put the Twiggy Cracked Tear in your Physick (no rune loss on death), drink it, and kill both knights. Then let the two regular soldiers at the north entrance kill you, and resurrect at the nearby stake of Marika located just outside the south entrance to that same camp. Rinse and repeat until you have the helm, or any other piece from this armor set.
