AnonymousCompletely missed this one. The Tombsward Cave entrance is in the cliff face Southeast of Fourth Church of Marika, and the Cookbook is in a tunnel to the right at the end of the first area of poison. Guarded by a pack of rats and a large rat at the end of the first stretch of this tunnel.

the map location is entirely incorrect, the real place for the item us in the tunnels slightly west of the Fourth Church of Marika underground in Tombsward Cave

I was under the impression that this was in Tombsward Catacombs, but its a totally different cave.
