- Option 1: After you defeat the Fire Giant boss and approached the Forge of the Giants. Return to Roundtable Hold, found on D's corpse.

Apparently there are several possible triggers that further this quest line to get to the point where you receive this object. For me, when I received the item that triggers this part of the quest from Fia, I did not have a second great rune (only Godrick's) but I did explore a lot of the map (basically entirely explored Limgrave and Liurnia, got into Raya Lucaria but did not start that legacy dungeon, and I explored a bit other higher level areas you get to via teleport).

I found this on D's body (with the deathbed companion next to it) without ever having seen the Fire Giant Boss or discovered the Forge so that can't be the (only) trigger. The last thing of any note I did was enter Ranni's service.

I got this item by completing Fia's dagger quest. After talking with her several times she eventually gives you a dagger. I gave the dagger to D who eventually opens a new door in Fia's wing where she appears to be holding D. One can loot D's body for this and other items.
