Kingsrealm Ruins is a Location in Elden Ring. The Kingsrealm Ruins is found in the northwest of Liurnia of the Lakes. The area is easily accessible on foot or on horseback by following the main road along the western landmass.

Kingsrealm Ruins Walkthrough
Kingsrealm Ruins hosts a hidden, underground area where the optional boss Royal Revenant can be fought. The stairs leading down into this area is covered by an illusory floor. You can find this in the northeast section of the ruins. The floor needs to be rolled on to dispel the illusion or hit with a melee weapon to be revealed.
Royal Revenant is a very quick and agile multi-limbed creature that can teleport around the arena and release a toxic breath in a long, straight line ahead of it.
Defeating the Royal Revenant does not drop anything other than runes. However you do gain access to the small room at the end of the hall. There is a chest inside that contains the Frozen Needle rapier.

Just wanted to mention, because I didn't see it on the page, there is an illusory wall in the middle back wall. It will let you continue forward past the church.

Didn't even know these were hidden. I think I must have unknowingly uncovered it while attacking one of the aristocrats. I died and returned to get my runes saw the stairs leading down. The only hint, if you could even call it that, is that the door leading the Iji and the bonfire is an illusory wall so MAYBE you'll think to look around for more illusory stuff. I don't know I wouldn't have found it had I not accidentally uncovered it. I would have probably just checked on here eventually and went back.

Use Margit shackle to reveal any hidden wall/entrance, this included, from 1km away.

Look miyazaki i love you but this hidden room is just not it. Theres almost nothing pointing out that there may be a hidden floor in this specific spot and unless someone feels like the area is too empty they wont even bother with searching this place

Killing the boss here has stopped me from getting any invaders even with taunters tongue in the entire Liurnia region

Cast Heal near the boss and it's trivial. With minimum faith it took me 2 casts to kill the boss.

Somehow none of the directions in other comments made sense to me, so here’s a better reference point:
Stand where the ghost/spirit is kneeling in the corner and face northeast. Take 5 steps forward, then turn right/east and walk through the hole in the wall. Keep walking forward/east down the steps, pass through 2 bushes until you stop at a low wall. It’s maybe 20-30 feet from the top of the steps. You’ll see a tree on your right and tiny gold flowers growing out of the wall on your left. Jump over this wall, and now you’re standing on top of the hidden area.

You can summon Torrent while fighting the boss. He's fast so it's good to outpace him, even in close-quarters.

If you look on the map, just above the icon of the Kingsrealm Ruins is the entrance close to a half-broken wall

Okay to find the hidden entrance just head north in the ruins following the path untill you reach a dead end, there's an illusionary wall there hit it and find the site grace behind it.
Now turn around and from the arch head south-east untill you find another arch (a magic trap will crumble the arch)
To your right you'll see a boulder and just a few paces past it is the entrace. Just roll over it.

How to actually find the underground entrance:
Start in the very northeast corner.
Turn a complete 180 backward.
Walk a few steps until you hit a grassy ramp.
Follow the ramp up west/right.
Walk until you hit a floor of bricks.
Jump strike the middle of that brick floor.
There you go.

At the edge of the ruins their is a huge arch towards the edge the trapdoor can be revealed when rolled on and is near that arch. Closer towards the lake

There is a illusory "floor" in the southern part of the ruins still kinda hard to find.

I knew there had to be something other than noble sorcerers here, but never found the hidden room
