Rivermouth Cave is a Location in Elden Ring, added with the Shadow of the Erdtree DLC. The Rivermouth Cave is found in Gravesite Plain region. To get to Rivermouth Cave, drop down the gravestones on the wall in Gravesite Plain to get to the bottom, then follow the wall going west until you get to the entrance of Rivermouth Cave. [Map Link]

Rivermouth Cave Walkthrough
As you enter Rivermouth Cave, you will immediately discover the Rivermouth Cave Site of Grace [See Elden Ring Map]. If you go to the cavern going east, you will encounter 4 wolves in an area. In the northeast side of the room, you can pillage a body for x2 Hefty Beast Bone.
As you head deeper, the path will fork. The path on the right will have two hostile wolves along with a dead body that you can loot for x3 Sliver of Meat. This leads to a dead-end, so return to the path and follow the other path. This path will lead you to an area with an upper level and three wolves.
If you climb up the opening on the east side of the area, you will find a body containing loot. When you get closer to loot the body, the ground will collapse, making you fall. You can then loot the body to pick up a Shadow Realm Rune [1]. The path going northwest will be a dead-end, so go down the path east. As you are traversing the eastern path, you can gather x1 Sanguine Amaryllis along the way.
The path will lead you to an area full of Bloodfiends. If you want to go back up, go to the path on your right, and you will encounter a Bloodfiend. After disposing of the enemy, follow the path west going up, in which you will encounter two wolves. Keep following the path, encountering two more wolves along with a dead body that you can loot for x1 Sacred Bloody Flesh. Keep following the path, which is going north, and you will be back in the area before you fell.
If you wish to continue, you can clear the area by sneaking up on the ogres and backstabbing them. They are also easy to deal with, as you can kill them in around two hits. If you aggro them all at the same time, you can fight one of them at a time by dragging the enemies away from the area. After clearing them, you can gather a few Sanguine Amaryllis around the edges of the blood pool. On the altar, you can pick up x3 Sacred Bloody Flesh. In the northern corner of the area, near the ramp where you came from, you can pick up x6 Sanguine Amaryllis. There will be a boss fog nearby.
When you enter the boss room, you will be facing the Chief Bloodfiend. It is a more aggressive version of the Bloodfiends that you encountered earlier. It can jump towards you from afar, while also using its club more effectively than the ones you encountered earlier. For a more in-depth guide on how to defeat the Chief Bloodfiend, you can go refer to this page. Once you have defeated the Chief Bloodfiend, it will drop x1 Bloodfiend Hexer's Ashes. You can find the exit on the southeastern side of the room.

AnonymousThere's a tunnel past a lake North of Moorth Ruins on Scadu Altus with a very tiny erdtree that leads to a poisoned lake, then head west into a huge tunnel, past the two golems, and you arrived at temple town ruins. (It took me hours to figure it out)

From the Temple Town Ruins site of grace, go south until you reach the cliffside. There's an altar with a scadutree fragment near where the gravestones are that let you descend into the river. They look like weird teeth on the map.

Use assasin's gambit and hug the left wall to avoid aggroing any enemy. Beware of the guy patrolling on the left, Move when he is going to the right. Also, you can pick up the item behind the big hexer guy, no one will see you :)

In the second room with the wolf ambush, can you get up to the level where one of the wolf jumps off from?

you do not need to drop down the gravestone wall to get to here. instead, you can take an (admitted circuitous) route by scaling down the rocks you see behind thiollier's original location at pillar path cross. following the water takes you to the ellac river cave-- from there you can keep heading north toward rivermouth cave. useful if you haven't gotten to scadu altus or just don't want to go there yet!
For such a hidden dungeon this really needs to have better rewards
