Painful Strike is a Skill in Elden Ring. It was added with the Shadow of the Erdtree DLC. Painful Strike is a punishing strike with the grotesque Tooth Whip that poisons the victim.
How to get Painful Strike in Elden Ring
- Default skill of the Tooth Whip. The weapon can be found inside the Whipping Hut. [See Shadow of the Erdtree Map]

- This is a Unique Skill
- FP Cost: 16
- Deals 14 stance damage
- This Skill is not Chargeable
- This Skill cannot be Parried
- Performs a flurry of whip strikes that causes Physical Damage and inflicts deadly Poison buildup
- Applies a Debuff to enemies which decreases their stamina recovery speed by -8/s (-17.7%)
- Duration: 30 seconds
- Its animation is the same as the skill Impact from Dark Souls 3, a skill all but 2 whips share

I don't really know if being hit by a whip made of teeth would be more painful than being hit by a normal whip to be honest

truecombos off a healthy range of offhand options, but also from it's own whip moveset:
1 handed: R1 1, R1 2, R1 3, charged R2 1, jump R1, jump R2
2 handed: R1 1, R1 3, *crouch R1*, charged R2 1, running R2, jump R1, jump R2
paired: jump L1
if you only hit with the last part of Painful Strike, it can truecombo into itself again.

The only painful thing about this is that it's locked to a unique weapon instead of being an ash of war.
